A. Collection of Personal Information
We collect personal information such as, name, age, sex, birthday, picture, nationality, citizenship,
religion, occupation, current employment, address, email address, telephone or mobile number, current
medication or treatments, past/present medical history, family medical history, name of the Insurance, or
Health Maintenance Organization (HMO) you are using, the name of your company especially for
corporate accounts, name of any health service provider or medical specialist if applicable, results of any
diagnostic test which you provide, and any other information that will aide us in providing you with quality
health care. We also get medical information through your medical providers who endorsed your care,
results or documentation we get from the diagnostic tests that you have done with us, forms that you
have filled out as requested by our allied health staff, medical staff or support staff and those whom the
company has authorized.
B. Usage of Your Personal Information
Personal data collected shall be used in providing medical services to you. It may also be used for billing
and collection purposes, and in processing your payments.
We would also use your personal information to liaise with health care professionals for coordination of
your care, and to your Insurance Company or Health Maintenance Organizations, and/or your Company if
under Corporate Account (eg: Annual Physical Exam, Pre-Employment Physical Examination, and
Executive Check Up).
We may also use your information for research to improve patient care outcomes, improve our delivery of
healthcare, and for other purposes as required by law.
C. Storage, Retention and Destruction
This company will ensure that personal data under its custody are protected against any accidental or
unlawful destruction, alteration and disclosure as well as against any other unlawful processing. The
company will implement appropriate security measures in storing collected personal information,
depending on the nature of the information.
We shall continue to keep your medical records for as long as you regularly avail the services of the
company. If the medical records reached a one-year (1) the hard copies shall be transferred at a secure
off-site storage facility. Inactivity of five-years (5) from the last out patient consultation these hard copies
shall be destroyed with reasonable and appropriate security measures. All electronic medical records
shall be retained following the above schedule, afterwards it shall be deleted or archived.
D. Access
Due to the sensitive and confidential nature of the personal data under the custody of the company, only
the client/patient, authorized representative of the client, the authorized representative of the company,
medical and allied health professionals who are involved in providing services to you, shall be allowed to
access such personal data, for any purpose, except for those contrary to law, public policy, public order or
morals.

We are committed protecting you’re the personal information that you have entrusted to us. The following

F. Disclosure and Sharing
All employees, allied health staff, medical staff, support staff and affiliates of the company shall maintain
the confidentiality and secrecy of all personal data that come to their knowledge and possession, even
after resignation, termination of contract, or other contractual relations. Personal data under the custody
of the company shall be disclosed only pursuant to a lawful purpose, and to authorized recipients of such
data.
We may also share your personal information with service providers acting on our behalf and have
agreed to keep secure and keep confidential your personal information.
We shall also share your personal information with HMOs and/or Companies with whom you are affiliated
with, and with whom you consented to the sharing of your personal information.
G. Protecting your Personal Information
1. We only access your personal information to the extent that is necessary as stated above in the
“Usage of Personal Information.”
2. Restricting access to your personal information only to authorized personnel.
3. By using a secured server that is being maintained with a firewall, anti-virus and other security
controls that are necessary.
